Community Health Systems Registered Nurse Navigator - Oncology 1.0 (rotating) in Valparaiso, Indiana
The Nurse Navigator is responsible for functioning as a part of multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients. Currently licensed in the State of Indiana as a Registered Nurse and Oncology Nurse Certification. Minimum two years related clinical oncology experience. Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the cancer patient from screening to survivorship. They will coordinate patient care throughout the continuum in collaboration with the multidisciplinary team. They will serve as a clinical resource with expertise in hematology/oncology care management. The Nurse Navigator will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population. They will provide expert nursing care, which includes: direct clinical practice, consultation, education and research.
- An Associate or Bachelor's degree in nursing from an accredited university or college. [Diploma Required]
Current IN unencumbered Registered Nurse license.
Maintain Basic Life Support (BLS/CPR) Certification.
Minimum two years clinical oncology experience.
